Have you had ongoing pain, or pain that comes and goes, for more than 6 weeks?
Did your pain originate without an obvious injury? Or, did it start after a minor physical stimulus that should not have been significant enough to cause chronic pain?
Do you sometimes or always notice your pain less when you are exercising, moving, or engaged in activity?
Have you been told surgery won’t help, or isn’t sure to help? Or that you have a problem originating in your nerves, muscles, or tendons?
Did you start with pain in one place, but now you have pain in two or more places?
Do you experience pain in the same place(s) on both sides of your body, or in multiple places only on one side of your body?
Do you feel fear about the present and/or future when you are experiencing your pain?
Can you sometimes start to feel your pain just by thinking about it?
Did your pain originate during a stressful time? Think carefully, could be “good” or “bad” stress.
Does the intensity and/or location of your pain change – throughout the day, week, or month?
Does your pain intensity change when you’re in a new emotional or physical environment, or when you’re distracted? Examples: a fun dinner with friends, or a walk in the woods.
Can you identify things that trigger your pain to come back or get worse – like the weather, stress, certain movements, locations, or activities?
